How can insulated wires be modeled with MMANA? Say a 2 mm diameter copper wire covered with PVC insulation, relative permittivity (dielectric constant) 3.5 and thickness 0.6 mm.

Use NEC-2 for MMANA by UA3AVR. It can open *.maa files and calcullated wires in dielectric isolation.
